Hospital Stay Discharge Options
Discharge to Home without Help
Patients are a candidate if:
- They have someone at home to help 24 hours a day
- They can move about their home and use the stairs if needed
- They have transportation to outpatient therapy three times a week
- They can faithfully perform their exercises twice a day without help
Discharge to Home with Home Health
Home Health nurses visit two or three times a week to check a patient’s wound and monitor any medical problems. A visiting therapist will also come to instruct and monitor exercises. Patients are a candidate if:
- They can get in and out of bed without assistance
- They can move about their home without assistance
- They do not have a way to leave their home for outpatient therapy
- They can faithfully perform their exercises twice a day without help
Discharge to Transitional Care Hospital-Based Skilled Unit
Patients are a candidate if:
- Their medical condition requires daily physician visits and skilled rehabilitation care
- They require assistance to get in and out of bed or to walk
- They are able to participate in daily therapy sessions
- They have the potential to be independent five – seven days after surgery
- Their insurance provides coverage and authorization for inpatient rehabilitation care
Discharge to an Extended Care Facility
Extended care facilities (nursing homes) are able to provide rehabilitation therapy once a day and assist patients with the activities of daily living. Patients should be able to return home quickly, pending their progress with therapy and independent activities. Patients are a candidate if:
- They live alone, or their family is unable to help 24 hours a day
- They require assistance to get in and out of bed or to walk
- They require assistance with bathing, toileting or meal preparation
- They will need two weeks or longer to regain independence
- Their insurance provides coverage for an extended care facility
